Sickness Report any sickness to your University Admin and follow sickness absence procedure. Absence Management Policy : Surrey and Borders Partnership NHS Foundation Trust (sabp.nhs.uk) If you are off sick for 3 occurrences in 6 months, you will need to have a Stage 1 Trigger Meeting with your University Tutor - this is a supportive meeting to see if there is any additional help/support required e.g. referral to Occupational Health. Mitzie will be informed of any Stage 1 Trigger Meetings and will support as required.

Immunisations

As you are an SABP employee, you will have the opportunity to arrange your immunisation/ vaccination with Optima. These will be available for you to attend at various sites in Surrey and will require you to book them directly with Optima. If you require information regarding your immunisation record you need to complete a Subject Access Request Form (see Appendix 1) (Occupational Health records have been transferred from SABP to Optima – unless you objected). This form needs to be returned to MedicalInConfidenceR@optimahealth.co.uk Your assignment/employee number is on your payslip or can be found on ESR. This process could take up to 4 weeks. It is your responsibility to save the details of your vaccination record. Please note when you have completed your studies and work at other Trusts you will be required to evidence your vaccination record.
