SABP Placements Feedback
If you do a placement with SABP you and your Supervisor will be required to complete a Feedback Questionnaire via our TCP Feedback App - you will be sent a link towards the end of your placement to do this. Please complete this via the app before your placement ends. Declaration of Interest Form Although not recommended due to the demands of the course, if you are working elsewhere you will need to complete a declaration of interest form -see Appendix 8 You need to fill in this form detailing the additional income and hours worked outside of your main job and that you believe it has no effect on your studies. The form needs to be returned to Employment services Employment.Services@sabp.nhs.uk Please also inform your course tutor and Mitzie Turner Wells.
